Your child's health information is protected, and you have rights regarding how it is used.
Our responsibilities
We are required by law to maintain the privacy of protected health information (PHI), to provide you this notice of our legal duties and privacy practices, and to notify you if a breach affects your information.
How we may use PHI
We use and disclose PHI for treatment, payment, and health care operations, and as otherwise permitted or required by law. Other uses require your written authorization.
Your rights
You have the right to inspect and copy records, request amendments, request confidential communications, request restrictions, receive an accounting of disclosures, and obtain a paper copy of this notice.

Contact our Privacy Officer at office@lavieestbelletherapy.org. You may also file a complaint with the U.S. Department of Health and Human Services. We will never retaliate for filing a complaint.
